Enrolment Verification Request

Submitting an Enrolment Verification Request form will produce an Enrolment Verification letter.

For full-time studies, the letter will identify the students':

  • Full name
  • Student Number
  • Date of birth
  • Full-time or Reduced load
  • Registered semesters and dates
  • Length of program (in semesters and years)
  • Program credential

For part-time studies, the letter will identify the students':

  • Full name
  • Student Number
  • Date of birth
  • Course code(s): With start dates, end dates and total contact hours

Please complete and submit the form using one of the methods outlined below.

  1. Online: fill in the form using the boxes provided.
  2. Once completed, print the form and sign it in the space indicated.

If you are not currently enrolled in a full-time program there is a charge of $10.00 + HST = $11.30.
